HEALTH AND SOCIAL SERVICES

A network-wide transformation becomes real when it helps each team act better.

Between a province-wide direction and one person's particular situation, a decision travels across institutions, territories, managers and teams with different realities. Mosaïca helps preserve its meaning all the way to the point of service.

  1. Network direction
  2. Institution and territory
  3. Front-line management
  4. Team
  5. The person and their family
A direction that sharpens as it descends toward the encounter Five levels follow one another from top to bottom: the network direction, the institution and its territory, front-line management, the team, then the encounter with the person and their family. Each level is narrower than the one before and its markings become finer. A single line runs through them, shifting direction slightly at each passage; a gold impulse travels down it and lights up each level in turn, through to the point of encounter, shown as two arcs facing each other.

WHEN CONSISTENCY BECOMES A SERVICE ISSUE

When the same intention produces different decisions and different experiences.

  1. A common direction has to be applied in institutions and territories with different realities.
  2. Decisions are moving closer to the front line, but the room for decision is not always understood in the same way.
  3. Professions and functions do not always read the same situation with the same reference points.
  4. Managers have to coordinate, support and engage while services keep running.
  5. New people discover their role at the same time as professional codes that are rarely made explicit.
  6. A patient or a family member may interpret words, silences and institutional habits differently.

One intention. Five places where the network experience is built.

01

The network

The network sets the broad direction, the commitments, the expected outcomes and the principles that have to stay recognizable everywhere.

What has to be recognizable across every part of the network ?

02

The institution and the territory

Each institution translates the direction according to its missions, its populations, its facilities, its partners and the particularities of its territory.

What has to be adapted to local reality without changing the meaning of the common direction ?

03

Front-line management

Managers turn direction into priorities, decisions, explanations and real conditions for action. Proximity becomes concrete in the clarity of their power to act.

Which decisions can actually be made closest to the person ?

04

The team and the professions

Clinical, psychosocial, administrative, technical and support teams bring together distinct bodies of knowledge, responsibilities and professional cultures.

Do the professions have enough shared reference points to coordinate their action ?

05

The encounter with the person

The patient, the service user or their family does not encounter the network's structure directly. They encounter a word, a decision, a wait that is explained, an attitude and a way of taking their situation into account.

This is where the transformation becomes a real experience.

WHAT TRANSFORMATION AMPLIFIES

The network and the territories

A common direction has to stay recognizable across institutions, facilities and territories with different realities. Proximity calls for a capacity to adapt. Consistency requires that the limits of that adaptation be clear.

One common direction expressed in three territories A horizontal line stands for the common direction. Three trajectories descend from it toward three different local forms, none a copy of another. In each, the same gold reference point remains present: what has to stay recognizable everywhere.

Professional cultures

Within the same care pathway, several professions bring their knowledge, their obligations, their language and their way of defining priority. The issue is not to erase those differences. It is to organize the places where they have to understand one another and decide together.

Four professional perspectives and their point of coordination Four trajectories coming from different heights cross a shared space, meet around a point of coordination marked in gold, then each continues on its own way. None merges into another.

  2. Psychosocial perspective
  3. Management perspective
  4. Support perspective

Reference points within the relationship

A silence, a direct question, a family member's place, a piece of feedback or a hesitation can carry several interpretations. Cultural intelligence makes it possible to suspend quick judgment, check understanding and adjust the relationship without giving up professional standards.

The same signal, three possible readings, one clarified reference point The same starting signal passes through three neutral reading frames and produces three slightly different responses. A moment of clarification then brings them together around a shared reference point. No frame is associated with a particular origin or colour.

WHAT WE BELIEVE

Front-line management cannot be decreed. It has to be made possible.

It becomes real when responsibilities are clear, managers are supported and teams have shared reference points for understanding, deciding and acting.

WHAT HAS TO STAY COMMON

The service commitments, the decision-making principles, the responsibilities and the behaviours that have to be recognizable throughout the network.

WHAT HAS TO BE DECIDED CLOSE TO THE PERSON

The adaptations that take account of the situation, the territory, the team and real needs, within clearly defined room for decision.

WHAT HAS TO BE SUPPORTED BETWEEN LEVELS

The trade-offs, the handovers of information, the feedback and the translation of direction when several responsibilities meet.

Mosaïca follows the path from direction to encounter.

We do not assess clinical acts or regulated professional practice. We examine how a direction is understood, translated and applied across structures, managers, teams and real situations.

  1. Read

Analyze the direction, the responsibilities, the processes, the coordination mechanisms and the expectations set for managers.

  2. Compare

Examine how the same intention is understood and applied across different institutions, territories, functions, professions or teams.

  3. Clarify

Determine the shared reference points, the legitimate adaptations, the responsibilities to clarify and the situations calling for arbitration.

  4. Embed

Turn the observations into management practices, tools for passing things on, integration journeys and mechanisms usable day to day.

Organizational culture · Cultural intelligence · Management practices · Onboarding, integration and retention

Between the network's direction and the encounter with a person, meaning travels through several levels. Mosaïca works so that it stays understandable, consistent and applicable.

THREE WAYS TO ACT

Step in where the direction changes meaning.

CHOOSE AN APPROACH

ORGANIZATIONAL AND RELATIONAL CONSISTENCY DIAGNOSTIC

See where the direction stays clear, where its interpretation shifts and where a decision becomes necessary.

We compare how the same direction is understood and applied at different levels of the network. The diagnostic brings out the shared reference points, the significant variations, the responsibilities to clarify and the situations calling for arbitration.

The diagnostic covers organizational, relational and managerial dimensions. It is not a clinical assessment.

A reading that cuts across the five levels and flags three variations Five horizontal levels are crossed by a vertical line of analysis moving from left to right. On three of them, a gold segment flags a variation in interpretation; those three flags are then connected to form the reading of priorities.

ALIGNING LEADERSHIP, MANAGERS AND TEAMS

Build shared reference points that genuinely support front-line decisions.

We bring together the people who define, translate and apply the direction. The work clarifies what has to stay common, what can be adapted, each level's responsibilities and the mechanisms that carry the reality of the field back up.

Four levels reaching a common base Four trajectories start from four different levels, each keeping its point of origin. They descend toward the same common base, where four gold markers confirm the alignment. No path disappears.

DEVELOPING MANAGERS, TRAINERS AND TEAMS

Equip the people who translate the transformation into daily practice.

We build their ability to manage on the front line, to set clear expectations, to give feedback that lands, to collaborate across several professional cultures and to act with precision in culturally diverse situations.

The programs can also support the people who welcome and train new recruits, so that the essential reference points do not rest on informal experience alone.

A reference point made explicit, then shared by a whole team A reference point crosses three professional environments along a link that is uncertain at first. Someone responsible for passing it on steps in from above and makes the reference point explicit: the link becomes continuous, and the reference point then spreads to a whole team, whose network stays visible.

EXPERTISE ALREADY AT WORK IN THE NETWORK

Create an internal capacity that stays after the engagement.

Mosaïca presented its Train the Trainer program to a CIUSSS and equipped managers across the network through the Mes Repères Professionnels program.

These initiatives pursue a lasting intention : enabling people inside the organization to pass on consistent reference points, to support new recruits and to act with greater precision in professionally and culturally diverse situations.

Our contribution does not concern clinical acts. It concerns what allows teams to act together : organizational culture, cultural intelligence, onboarding, knowledge transfer and management practices.

A network becomes more human when each level helps the next one act better.

From province-wide direction to the encounter with a person, consistency has to stay visible.

Mosaïca helps organizations close the distance between their intentions and what people experience day to day.
